Treating Premature Ejaculation

How is Premature Ejaculation Treated?
The main concern in PE is the loss of control. Treatment is aimed at helping the man maintain his erection, improve his confidence and control his ejaculation. When he is able to focus on the enjoyment of sex and not on the anxiety of premature ejaculation, the sexual experience will be more fulfilling and he will be better able to satisfy his partner.

  • Oral medications are often used to delay ejaculation. Physicians who are experienced in sexual medicine may prescribe low-dose antidepressants off-label, since this class of medications is known to safely and often effectively delay ejaculatory response. Other less experienced doctors may avoid prescribing antidepressants for PE since it has not been specifically approved for that purpose. Oral medication may not be appropriate for everyone and certain medications in this category are more favorable than others. It is therefore very important to seek the help of a physician who has experience treating PE since the experienced doctor is more likely to understand and potentially prescribe these treatment options, and will be more likely to offer other treatment options when oral medication isn’t suitable. Important studies are currently underway which may lead to the approval of the first oral pill specifically indicated for the treatment of PE. It is believed that the investigational drugs currently being studied are even more suitable for the treatment of PE than other oral medications currently being used.
  • To reduce over-sensitivity, topical creams that mildly numb the penis can be prescribed. When used properly, this can help the man to delay his ejaculation. If used improperly, these creams may also numb the partner, thus decreasing the partner's sensation. Thus, it is important to receive instructions from an experienced professional on how to correctly use these creams before trying it on your own.
  • Sexual Counselling is incorporated in the treatment of PE. Counselling may include the teaching of behavioural methods (special penile exercises), which are taught in the clinic, but practised at home. These techniques can help “train” the body to control ejaculation and prolong the lovemaking experience. For a man who does not want to use any medication, these techniques could be used on their own, however for better results, a combination of pharmacotherapy and behaviour methods should be used. Sometimes psychotherapy or other types of talk-therapy may be required to explore deep-seated anxieties of sexual relationships.
  • Self-injection therapy can also be used in the treatment of premature ejaculation, often in combination with other options such as behavioural methods and sexual counselling. This therapy ensures a prolonged erection, regardless of whether the man ejaculates or not, thus providing a sense of control. Sexual tension diminishes as the pressure to perform is no longer "in the way." Men develop improved self-esteem and confidence while the need to use medication is reduced and eventually eliminated. For some men treatment may take a few weeks, and for others it may take a few months. Success rates are very good.
